To enter, complete the Arkansas Medical Cannabis Patient Survey Sponsored by Arkansas Cannabis Industry Association by 10:00 p.m. on Monday, January 14, 2019.
It’s that simple. The contest will end at 10:00 p.m. on January 14, 2019.
1. THE CONTEST: Via the Arkansas Cannabis Industry Association Patient Survey.
2. THE ENTRY PERIOD: The entry period for the contest will begin on January 4 at 6:00 p.m. Central Time and will end on January 14 at 10:00 p.m. Central Time.
3. ELIGIBILITY: The Competition is open to persons who legally reside in the United States who are 21 years of age or older at the time of entry. Arkansas Cannabis Industry Association and each of their affiliates and employees, as well as the immediate family (spouse, parents, siblings and children) and household members of each such retailer, affiliate, or employee, are ineligible to participate in the contest.
4. HOW TO ENTER: Complete the Arkansas Medical Cannabis Patient Survey Sponsored by Arkansas Cannabis Industry Association by 10:00 p.m. on Monday, January 14, 2019.
5. PRIZE WINNER SELECTION: One winner will be selected at random among pool of entrants by the contest Sponsor, Arkansas Cannabis Industry Association, within one day after the close of the Entry Period. The Sponsor will notify the winner via email. The Sponsor will award one Vaporbrothers VB1 Vaporizer to the winner. Winners will receive prizes within one week of selection, subject to availability.
6. ALTERNATE WINNERS: If a winner is found to be ineligible, then another winner will be selected from the pool of entrants. The potential winners will not receive the prize until his/her eligibility and identity have been verified. This process will be repeated until all winners are verified.
7. RELEASE: To the fullest extent permitted by law, in exchange for the opportunity to enter and participate in the competition and win the prize, you, for yourself, your heirs, next of kin, administrators and executors, forever waive, release and hold harmless (“RELEASE”) any and all rights, claims, damages, liabilities, costs, attorneys’ fees, expenses and causes of action whatsoever (“CLAIMS”) that you may have against the sponsor, the competition vendors and suppliers, and its affiliates, employees and agents, vendors and suppliers, the judges, prize providers and each of their respective related entities (The “RELEASED PARTIES”) and each such released parties’ officers, directors, employees and agents. This release covers, without limitation, any claims arising directly or indirectly out of your entry into and participation in the competition, your status as an entrant, finalist and/or a potential and/or actual winner, and/or use or non-use of prize and includes, without limitation, claims based on personal injury, death, property damage, libel, slander, defamation, invasion of privacy/publicity, portrayal in a false light and any claim relating to the sponsor hereunder. Further, you hereby release the released parties from any third party intellectual property claims arising from or in connection with your entry. This RELEASE survives the expiration of the competition.
8. PUBLICITY: Except where prohibited, participation in the contest constitutes your consent to the Sponsor’s use of your name, likeness, voice, opinions, biographical information, hometown and state for promotional purposes in perpetuity in any media, worldwide, without further payment or consideration to you and without obtaining any prior approvals.
9. SURVEYMONKEY DISCLOSURE: This promotion is in no way sponsored, endorsed or administered by, or associated with, SurveyMonkey. You understand that you are providing your information to the Organizers and not to SurveyMonkey. The information you provide will only be used as outlined above in the Official Rules.
